Effect of Weight Loss on Cerebrospinal Fluid and plasma Concentrations of NPY, Galanin, CRH and Leptin in Obese Women

Abstract
Numerous hypothalamic peptides are involved in the regulation of food intake and body weight. NPY and galanin exert potent orexigenic action, while CRH and leptin exert an opposite action in animal models. The objective of this study was to investigate the effect of weight loss on cerebrospinal fluid (CSF) and plasma levels of NPY, galanin, CRH and leptin in 14 normal weight women (mean BMI 20.4 kg/m2) and 16 obese women (mean BMI 35.6 kg/m2) before and after weight loss by 2 week-very low caloric diet (800 kcal/day). In obese subjects, plasma leptin and, CSF leptin and galanin levels were significantly increased compared with normal weight subjects. No significant differences were observed in CSF NPY and CRH levels as well as plasma NPY, CRH and galanin levels between obese and normal weight subjects. CSF leptin and galanin levels were positively correlated with body mass index. After 5.3 % weight loss, CSF NPY, CRH and leptin levels significantly decreased in obese subjects, while CSF galanin levels did not change. The degree of weight loss was negatively correlated with the baseline CSF NPY level (r=-0.74, p<0.001). The baseline CSF NPY level was positively correlated with CSF galanin (r=0.78, p<0.001) and CRH levels (r=0.63, p=0.03), whereas it was negatively correlated with the level of CSF leptin (r=-0.69, p=0.03) in obese subjects. After weight loss a significant negative correlation was observed between the change in CSF leptin and the change in CSF NPY (r=-0.81, p<0.001).
